11th Clinical Research Conference

"The future cannot wait for Greece"

Rapid developments are foreseen in the near future in the clinical research sector on an international level. The New European Regulation, the increasing use of digital technology, and Decentralized Clinical Trials seem to be the drivers behind these developments.

Greece is still lagging behind in this respect, despite the government’s recent steps in the right direction. For example, a ministerial decision lifted a major counter incentive for hospital administrations and now the revenues from clinical research are calculated separately from state subsidies. However, that is not enough. Coordinated efforts are required to cut through the red tape once and for all at a hospital level, and executive training is needed to change the culture on clinical research at the National Health System (ESY). In a move in this direction, the decision has been made for special offices with suitably trained personnel to be created at 15 major hospitals.

All the latest developments on clinical research will be discussed at the 11th Clinical Research Conference, the hybrid event to be held on December 13 at Divani Caravel Hotel. The conference is being organized by ethosEVENTS in partnership with the health information portal virus.com.gr and health policy magazine Pharma & Health Business.

Coordinator of the conference will be Mr. Emilios Negis, journalist and editor of Pharma & Health Business and Virus.com.gr.

Closed (Hybrid) Lobbying Workshop

During the works of the 11th Clinical Research Conference a Closed Lobbying Workshop will be held with the participation of the General Secretary οf Health Services, Mr Ioannis Kotsiopoulos, hospital directors and executives, and Health Regions from all over Greece.

The workshop is organized as a hybrid event, with physical presence of participants and top executives of the conference sponsors, while the option of digital attendance will be available through LiveOn Expo Complex.

The 11th Clinical Research Conference includes important speeches and panels with the following key matters.

Panel 1: Measures and policies for the promotion of clinical research in Greece.

Despite the pandemic, there has been an increase of approved clinical research in Greece over the past 3 years. Nevertheless, Greece still ranks near the bottom in the EU, mainly due to bureaucracy at a hospital and Health Region level.Currently, clinical research with a total budget of approximately €80 million is approved in Greece annually. Unfortunately, however, we have no reliable data regarding the research that is ultimately carried out and the amounts absorbed on an annual basis.On an EU level approximately €35-€40 billion are invested annually in clinical research. If we take into account the level of the scientific personnel, which remains of a high standard despite the problems being faced, the size of country, and the health system, then it is estimated that Greece could attract 4 or even 5 times more resources for clinical research.Specialists note that we need to simplify processes and utilize new technologies. We need a change of culture at hospitals, and director and employee training on the value of clinical research is required.

Recently there was an important provision in the right direction. The new provision states that the amounts recorded as income at hospitals from clinical research will be in excess of the appropriations that have been provided for in the state budget.

The necessity to remove this counter incentive had been highlighted at the committee that had been formed regarding this matter last year under secretary general Mr. Giannis Kotsiopoulos. At the same time, the Ministry of Health is promoting the creation of special clinical research offices at 15 major hospitals, although there have been delays in the implementation of this measure.

The pandemic brought about changes in the implementation of clinical research thanks to the use of new technologies, which are here to stay. Experts note that it is a golden opportunity for Greece to invest in the utilization of technologies such as Artificial Intelligence and Big Data. At the same time, they underline that Greece needs to focus on specific therapeutic fields, such as hematology and oncology.

Panel 2: New European Regulation: Prospects-opportunities-impacts.

On January 31, 2022, the new EU regulation on clinical trials was implemented, harmonizing the procedures for submission, evaluation, and supervision in all member-states.In that same framework the operation of the new Clinical Trials Information System (CTIS) will be launched. It will be the exclusive means by which sponsors and regulatory authorities will apply for and evaluate clinical data, while it will include a database search engine for health-care professionals, patients, and the broader public.As announced by EMA, a transitional period of 3 years is provided for, which means that by January 2025 all trials approved based on the current regulation will then be regulated by the new regulation, and must to have been moved to the CTIS.With the launch of the CTIS, sponsors will have the option of submitting clinical trial applications to as many as 30 countries in the EU and the EEC at the same time, using the same documentation. According to the EU, this will enhance Europe’s position in clinical research. At the same time, the public will obtain access to detailed information and the conclusions of all the clinical trials that are on the system.This new regulation is part of the Accelerating Clinical Trials in the EU (ACT EU) program for the 2022-2026 period. The goal is to further develop the EU as a clinical research hub, to promote the development of high quality, safe, and effective medicines, and to better incorporate clinical research in the European health system.

Naturally, three serious questions arise:

  • What stage are we at internationally?
  • Are we ready in Greece?
  • What is the impact on Greece?

Panel 3: The utilization of new digital technologies and decentralized clinical research.

A wealth of research indicates that traditional clinical trials are currently facing serious obstacles in registering patients, in long travel, or due to the time spent at hospitals or single-trial centers. For example, research has shown that 70% of participants live at least 2 hours away from the research center.One of the dominant new trends internationally in the clinical trials sector is the utilization of new technologies in carrying out decentralized research. This research utilizes digital technologies so that the entire research or part of it can be carried out from the patient’s home or from the office of a local doctor, instead of the main research center, such as a major hospital.The main advantage of decentralized clinical research that has been identified by researchers is ease of participation, as it allows for flexibility, it requires fewer personal visits to the hospital, it allows for passive data collection, and it removes obstacles to participation, such as location factors, available time, and travel.Furthermore, there are several aspects of added value to decentralized clinical trials, with regard to the research taking place.

  • Firstly, there is a better generalization of trial results, as decentralized research allows for different and representative cohorts of patients, thus improving the quality of the research.
  • Secondly, there is better patient commitment, as it allows participants to self-manage.

Other benefits include improvements in participant recruitment and retention, greater commitment and variety, and trial implementation cost reduction.

On the other hand, however, there is a significant disadvantage: Patient safety. There is a risk of physical harm to participants if they administer the drug under trial improperly or unsafely. Furthermore, the complexity of these remote clinical trials may lead to the patients misunderstanding the purpose of the trial, resulting in reduced compliance. This may ultimately lead to inaccurate research findings.
