Distortions Persist – Which Reforms Do We Need?

The medical devices market in Greece is facing significant challenges that affect both the sustainability of sector companies and patients’ unhindered access to innovative therapies.

Despite investment momentum and the introduction of innovative health solutions, companies in the sector are under severe financial strain:

  • Arrears / Outstanding Debts: Despite government commitments for a drastic reduction, hospital overdue liabilities closed in 2025 at €1.397 billion (up from €1.164 billion in 2024). Along with EOPYY’s €231 million, total healthcare debts reached €1.628 billion.
  • Uninvoiced Materials: A significant part of the problem consists of materials delivered to hospitals and utilized, which remain uninvoiced for long periods, severely weighing on companies’ liquidity.
  • Clawbacks: Automatic clawback mechanisms force companies to return massive percentages of their revenues to the state due to overruns of closed, inadequate public budgets.
  • Underfunding: While demand for modern therapies and diagnostic tests is growing at double-digit rates, public funding increases at an exceptionally slow pace.

Greece lacks an integrated and transparent Health Technology Assessment (HTA) mechanism for medical devices. Product procurement is carried out almost entirely on the basis of the lowest price alone, rather than the clinical added value of each product.

Regarding EOPYY, serious distortions remain:

  • The current reimbursement status for medical devices is bureaucratic and time-consuming, practically delaying Greek patients’ access to new, more advanced technologies.
  • Despite marginal corrections, budgets for categories such as diabetes consumables (sensors, strips) or medical supplies (dressings) fall significantly short of the population’s actual needs.
  • Since 2021, and in full effect during current years, EOPYY pre-collects 70% of the estimated clawback on a monthly basis during invoice settlement. This means companies are directly and automatically deprived of 70% of the funds exceeding the closed budget, even before the final semi-annual clearance takes place.

Despite efforts for centralized procurement through the National Central Health Procurement Authority (EKAPY), major tender procedures are significantly delayed, forcing hospitals into direct awards or summary/simplified tenders.

Recently, legislation was passed to integrate Military Hospitals into EKAPY. The goal is to consolidate purchasing across the full spectrum of public health structures to achieve better pricing and faster repayments through shared funds. However, there is reasonable concern that this regulation will further slow down the NHS’s (ESY) already time-consuming procurement procedures.

All the latest developments in the medical devices market will be discussed at the 12th MedTech Conference 2026 titled: “Distortions Persist – Which Reforms Do We Need”, to be held on July 13, 2026, by ethosEVENTS, in cooperation with Pharma & Health Business magazine and the portal virus.com.gr.
